This is the clinician view. It keeps every sentence of the plain guide and adds, for each section, the study design and citations of its sources. Every added sentence is cited and checked like the rest. Back to plain language.
What is alpha-gal syndrome?
The syndrome is caused by IgE antibodies against a sugar called galactose-alpha-1,3-galactose (alpha-gal). It can involve reactions to meat, organ meats, dairy, gelatin and other products, including medicines made from non-primate mammals.Review1
In the study that first described it, people reported anaphylaxis or hives 3 to 6 hours after eating meat. They reported fewer or no episodes when they avoided it.E2Good3
Their symptoms of anaphylaxis, angioedema or hives were linked with eating beef, pork or lamb.E2Good3
Blood tests found IgE to beef, pork, lamb, cow's milk, cat and dog, but not to turkey, chicken or fish.E2Good3

Why do ticks matter?
The syndrome is strikingly regional because tick bites play an important role in sensitization. It is more common in groups at risk of tick exposure.Review1
It has been reported on every continent except Antarctica. In the United States most reports are within the range of the Lone Star tick, from New York and Iowa to Texas and Florida.E2Good2

Can it cause only stomach symptoms?
Yes. A gastroenterology practice update describes a gut-only form with abdominal pain, diarrhea, nausea or vomiting, without mainly skin, breathing or circulation symptoms.E2Good2
Alpha-gal syndrome is increasingly recognized as a cause of isolated gut illness.Review1

What is not known yet
Alpha-gal sensitization has been linked with cardiovascular disease. A link is not proof of cause.Review1
Questions you might bring to your clinician
- Could delayed reactions after meals fit alpha-gal syndrome?
- Which mammal products beyond meat matter in this case, such as dairy or gelatin?
- Should medicines or medical products made from mammals be reviewed before a procedure?
- Is there a written plan for a delayed reaction?
Every claim in this guide was written from its sources and checked against them word for word. Its clinical review by Dr. Recio, whose specialty is anesthesia, is pending; an allergist has not yet reviewed it. This line changes when each review is done.
Alpha-gal syndrome (AGS) is a serious, potentially life-threatening allergy to alpha-gal that can develop after a tick bite.
Alpha-gal is a molecule found in most mammals, such as cows and pigs, but not in people.
Food sensitization means a person has a positive IgE test to a food but can still tolerate that food in their diet.
The terms allergy and allergic disease are broadly encompassing and include clinical conditions associated with altered immunologic reactivity that may be either IgE mediated or non-IgE mediated. IgE is a unique class of immunoglobulin that mediates an immediate allergic reaction.
Anaphylaxis is defined as a serious allergic reaction that is rapid in onset and may cause death.
Urticaria is a frequent, mast cell-driven disease, defined by a rapid appearance of wheals, angioedema, or both. The lifetime prevalence of acute urticaria is estimated to be approximately 20%.
